Patient notes raised doubts

HAROLD Shipman's medical notes were not only fabricated, but often illegible.

Prof Richard Baker described his record keeping as scant and difficult to read.

He said: "His writing was sometimes illegible, but of greater concern was the lack of detail he recorded about patients presenting complaints and his clinical management."

The University of Leicester academic drew attention to one comment recorded by Shipman on a cremation form, stating, 'Saw patient at home, diagnosis made, arranged ambulance, patient dead when went back, all within 10 minutes'.

The report adds it's sometimes unclear whether his records were merely inadequate or fabricated.
