THIRTY-eight families will never know if their loved ones were killed by Harold Shipman.
After examining evidence in 888 cases Dame Janet Smith gave 494 individual written decisions.
The inquiry has looked at 2,311 witness statements taken by the police and 1,378 from friends, relatives and professionals.
Since the opening of phase one of the inquiry in June 2001 Dame Janet has had to make decisions on each case based on a probability of guilt.
Using documents, oral evidence and expert witnesses the team has attempted to piece together the events leading up to each death.
But 38 cases remain a mystery, with few surviving documents or witnesses.
